ABSTRACT

OBJECTIVE:To observe clinical efficacy and safety of linezolid,teicoplanin and vancomycin in the treatment of hospital-acquired pneumonia caused by methicillin-resistant Staphylococcus aureus (MRSA). METHODS:120 patients diagnosed as hospital-acquired MRSA pneumonia were divided into linezolid group,teicoplanin group and vancomycin group according to therapeutic regimen,with 40 cases in each group. Linezolid group received Linezolid injection 600 mg,ivgtt,bid;teicoplanin group received Teicoplanin injection 0.4 g,ivgtt,bid;vancomycin group received Vancomycin injection 1 000 mg,bid,ivgtt. 3 groups received 2 weeks of treatment. Clinical efficacy and bacterial clearance effective rate of 3 groups were observed as well as serum levels of inflammatory factors before and after treatment. ADR of 3 groups were compared. RESULTS:The clinical effective rates of linezolid group,teicoplanin group and vancomycin group were 90.0%,72.5% and 67.5%;the effective bacterial clearance rates were 85.0%,60.0% and 57.5%,respectively. The clinical effective rate and the effective bacterial clearance rate in linezolid group were significantly higher than those in teicoplanin group and vancomycin group,with statistical significance (P0.05). There was no statistical significance serum inflammatory factors among 3 groups before treatment (P>0.05). CRP and PCT of 3 groups de-creased significantly after treatment,and those of linezolid group were lower than teicoplanin group and vancomycin group,with statistical significance(P0.05). There was no statistical significance in the incidence of ADR among 3 groups (P>0.05). CONCLUSIONS:For hospital-acquired MRSA pneumonia,linezolid is better than teicoplanin and vancomycin in pneumonia control with good safety.

ABSTRACT

Objective To explore the safety and efifciency of oral iron therapy and intravenous iron therapy in anemia patients with maintenance hemodialysis,in order to gain experience for clinical treatment of this disease. Methods 88 cases collected in the ifrst people's hospital of Changshu city from July 2010 to March 2013 were divided into research group(n=44) and control group(n=44) by random number table. Control group was given oral iron therapy, and research group was t given intravenous iron. The differences of hemoglobin (Hb), reticulocyte, serum ferritin content, superoxide dismutase (SOD) and serum malondialdehyde (MDA) before and after treatment between two goups were recorded and compared. Results The differences of Hb, serum ferritin and reticulocyte numerical before treatment between two groups were not signiifcant, but signiifcant four weeks after treatment, with higher level in research group(P<0.05).The difference of oxidative stress status before treatment between two groups was not signiifcant, but after treatment, the MDA level in research group was higher than control group(P<0.05),while SOD level was lower than control group(P<0.05). Conclusion Compared with oral iron therapy, intravenous iron has better results in improving anemia, but easy to induce oxidative stress reaction.

ABSTRACT

Objective To establish age related reference value for the new reticulocyte hemoglobin equivalent (RET-He) parameter in healthy children aged one to thirteen years. Methods Reticulocyte hemoglobin equivalent was measured by Sysmex XE-2100 after completing daily internal quality control in 424 healthy children and the outlying observations was handled afterward, Age related reference value anddistribution of RET-He level were described as the 2.5th and 97.5th centiles, and the equation regardingage related reference value was established through curve fitting methods. Results After grouping accordingto sex, the medians of boys and girls in RET-He were 31.30 pg,31.80 pg. Non-parametric test showed therewas no differences between the boys and girls. When the data from two group was integrated together, thefitted equations of RET-He were: P2.5 =-0.008X3+0.125X2 - 0.178X+26.456; P97.5= 0.021X2-0.184X+34.670(X refers to age) ; R2 were 0.85 and 0.90,respectively(P <0.05). There was statisticallysignificance for the above equations. Conclusions The reference value of RET-He in healthy children isdifferent from those in adult. The equations can be used to produce age related reference value. It could beuseful for anemia diagnosis, differential diagnosis and hematopoiesis screening at early stage.
